In 2022, a worker was fatally injured while performing work underwater. This presentation will review this incident, discuss why many of the choices leading to this incident seemed reasonable at the time, and what decisions could have resulted in a much different outcome. While this specific incident happened underwater, the lessons learned are directly applicable to the hiring of any specialized subcontractor. This presentation will also discuss a second incident which occurred in Alberta with similar themes.


These lessons are broadly applicable when hiring a contractor for any specialized work, including rope access, confined space entry, electrical work, work with explosives, or anything else requiring a very specific skillset.

  • understand the importance of following your organization's contractor vetting process
  • understand the importance of hiring qualified workers to do work
  • understand that money does not need to change hands for a contracting relationship to exist in the eyes of OH&S

About the presenter

Mark Faas is a PADI-certified scuba instructor, cave and technical diver, and professional engineer in Alberta. Mark was born into a diving family, becoming certified at a young age. His diving has taken him around the world. He enjoys exploring Alberta’s most difficult dive sites. He is also a mechanical engineer, working at a chemical plant to support his diving addiction!
